           The DDC is responsible for requesting and receiving all documentation to support the sampled claims for the PERM program.   If no documentation is received within the appropriate time frame, the claim is automatically counted as an error.

          Communication and cooperation with state Medicaid officials and providers is critical to the success of this program.  From this site state officials are able to monitor the progress of record request and receipt for the sampled claims in their state.  State assistance and follow-up with providers who have not responded is always much appreciated.   

        To access any information from this point you will need to have an assigned user name and password.  The reports you access are real-time, pulling from our internal database, so they are accurate up to the minute they are run.  Livanta does not post any reports to this website.
